Find a form

Completed forms allow us to take action on your behalf. Some requests may be completed online through our Personal insurance web portal; but for others (e.g., to change a name, or a payment amount), we need you to send in a completed form.

Those individual forms need to be downloaded from our site (Note: links to commonly accessed forms have been included below). For answers to questions about downloading forms (and completing them), please visit our general Find a form page.

Online Access

Many insurance policyholders can access their information, update an address, and more online.*

* At this time, corporately owned policies and policies issued by Standard Life are not accessible online. 

You can change your address and bank information using the forms below, and in many cases through an online account. (Please note: *At this time, corporately owned policies and policies issued by Standard Life are not accessible online).

  • To access a policy online, you must set up a Manulife ID, and connect your Manulife ID to your policy.
  • If you already have a Manulife ID, you don’t need to set up a new one for your insurance policy. You’ll just need to connect your insurance policy to your existing Manulife ID.
  • For more details about setting up or connecting your Manulife ID to your products or accounts, please visit the Personal insurance web portal.

Information on downloading forms

Here’s how to:

  •  get documents off our site (PDF technology)
  •  fill them out using Adobe Reader XI or higher or print them out and complete them
  • send them back to us

Click on a link to access a form

  • If it downloads automatically and opens as a PDF (in Adobe Acrobat Reader DC, Acrobat Pro DC, or Acrobat DC), you can start filling it out right on your computer. When you’re done, under ‘File’ on your screen (upper left corner of the PDF window), click ‘Save as’, give it a name, and save it to your computer. Then, print it off and sign it. Follow the mailing instructions included right on the form to send it back to us.
  • If it downloads automatically but isn’t visible, the form may be in your ‘Downloads’ folder. Check for it there, and when you find it, open it, and save it on your computer. Then, print it off and sign it. Follow the mailing instructions included right on the form to send it back to us.
  • If it doesn’t download, settings on your internet browser may need adjusting. Instructions for two internet browsers:

Scenario one:

  • Click on the three dots in the top right of your browser and click on Settings
  • Click on Privacy and security in the left-hand menu
  • Click on Site Settings
  • Go to Content > Additional content settings > PDF documents
  • Enable “Download PDFs” (Do not click on “Open PDFs in Chrome”

Scenario two:

  • When you click on a form link, it may download to the bottom left of your computer screen, sit on a download bar across the bottom, but not open.
  • In that case, click on the arrow on the download and choose “always open in Adobe Reader” or “System Viewer”.
  • Next time you download a form, it will open in Adobe Reader or your system viewer for better functionality.

Scenario one:

  • Click on the three dots in the top right of your browser and click on Settings
  • Click on Cookies and site permissions, then scroll down and click on PDF documents
  • Turn on “Always open PDF files”

Scenario two:

  • When you click on a form link, it may download to the bottom left of your computer screen, sit on a download bar across the bottom, but not open.
  • In that case, click on the arrow on the download and choose “always open with System Viewer”.
  • Next time you download a form, it will open in your system viewer for better functionality.

If applicable, you may need to contact your local IT department to make sure your web browser has been set up properly (note: in some cases your IT department has the Admin rights to allow you to change the settings on your computer.

Also, if when accessing a PDF file, you see a message that asks if you would like to open the document in a browser, you should always click ‘No’.

Many Manulife personal insurance customers can now access their insurance policy details online. But, details of policies that are corporately owned (i.e., owned by a business) or issued by the former Standard Life are not currently available online.

You can download the critical illness or disability form . For inquiries on critical illness, living care or disability claims call 1-888-626-8543 or email us at  CC_987@manulife.com

Fill out this form with as many details as you can and one of our associates will get back to you. Or you can call us at 1-888-626-8543 with the following details to submit your claim:

  • The policy number
  • The policyholder’s name and date of birth
  • The funeral home name (for life insurance claims)
  • Date of death
