The Association Health & Dental Plan has 8 different options for your clients to choose from — helping them find one that best suits their needs and budget. 

Most plans are guaranteed acceptance, making this the perfect sales opportunity if you have clients who want coverage without the medical exam.

Who it's for

If you have clients not covered by a group health and dental plan, are self-employed, or are looking for supplemental coverage, the Association Health and Dental Plan can be right for them.

This plan is also designed to offer small or large associations and small businesses with an individual health and dental insurance plan.

Base Plan

Guaranteed Issue Plan

Drugs: 70% of first $750, up to $525/year
Dental: 70% up to a $400/year
Vision: $100 every 2 years
Specialists/Therapists: $25/visit, up to $500/year
Psychologist/Psychotherapist: $80/first visit, $65/subsequent visits for max 10 visits/year
Travel (to age 65): 5 days

Bronze Plan

Requires Medical Underwriting

Drugs: 70% of first $500, 80% of next $2,500, up to $2,350/year
Dental: 70% up to a $500/year
Vision: $100 every 2 years
Specialists/Therapists: No limit per visit, up to $750/year
Psychologist/Psychotherapist: $80/first visit, $65/subsequent visits for max 10 visits/year
Travel (to age 65): 9 days

Requires Medical Underwriting 

Drugs: 70% of first $500, 100% of next $4,650 up to $5,000/year
Dental: 80% up to $600 in year 1, $900 in year 2 & beyond
Vision: $150 every 2 years
Specialists/Therapists: No limit per visit, up to $1,000/year
Psychologist/Psychotherapist: $80/first visit, $65/subsequent visits for max 12 visits/year
Travel (to age 65): 17 days

Requires Medical Underwriting 

Drugs: 90% of first $2,222, 100% of next $8,000, up to $10,000/year
Dental: 80% up to $750 in year 1, $1,000 in year 2, $1,200 in year 3 & 4, $1,500 in year 5 & beyond
Restorative dental: 60% in year 3 & beyond up to $800 every 2 years
Vision: $250 every 2 years
Specialists/Therapists: No limit per visit, up to $1,500/year
Psychologist/Psychotherapist: $80/first visit, $65/subsequent visits for max 15 visits/year
Travel (to age 65): 30 days
